Documents Needed for League Games and Tournaments

Several documents are needed for each league game. It is suggested that all paperwork is prepared at the beginning of the season so it is ready for each game.  The coach and manager should decide who will be responsible for game documents so there is no confusion during the season.

Documents Needed for Every League Game:
  • Player passes:  When these are received, each player/coach/manager should sign theirs, then the passes should be laminated.  Once laminated, it is easiest if these are on some sort of ring holder so they are easily accessed at games.
  • Competitive Match Report:  Complete this MYSA form online and save it to your computer so that it can be updated as needed.  Complete the player and coach information (registration # is on the player passes).  Game information can be typed in or hand written.
  • Medical Forms:  Copies should be kept in a folder or binder and brought with you to each game.
 
 
Documents Needed for Home Games:
All documents above, and:
  • Stamped, Addressed Envelope:  This envelope is given to the referees to send in the Competitive Match Report.  Address labels are provided in the MYSA coaches packet.
  • Cash Payment for Referees:  The Referee payment amounts are in a document titled Referee Verification Instructions and Fees under the Team Accounts tab to the left.
  • Referee Verification Form:  This form is signed by each referee before the game when payment is given.  Forms should be kept together and turned in to the referee coordinator at the end of the season.   

** The home team also supplies Corner Flags and the Game Ball.  Be sure to work out with your coach who brings these to the games.

 
 
For Tournament Check-in:
  • Travel permit (for out-of-state tournaments only)
  • Player passes (see above for details)
  • Competitive Match Report (bring one for each game)
  • Tournament Team Roster or Official MYSA Roster
  • Medical form for each player
**Tournaments may have unique forms and requirements, so check their website
**Have all documents and player passes in alphabetical order to streamline check-in.
 
 
 

 


Paying the referees

NEW FOR 2012:  There are new pay rates for the referees.  Everything you need to know about pay rates, payments, etc., is now printed on the Referee Verfication Form.  You need one of these forms for each home game.
