Frequently-used forms

Below are the few forms that you are likely to need.  Most teams get through the season just with the forms below, and almost none of our teams need the "Ejected Participant Report!"

TIPS about these forms:

  • MYSA Competitive Match Report
    At the start of the season, type in the player and coach data, but leave the game-specific fields blank.  Then save the PDF file, and print off enough copies for your home games.  On game day, you can handwrite the game-specific information, and for absent players you can simply draw a line through their names.

If your team has a paid coach/trainer

PAPERWORK
The law requires us to keep on file a withholding disclosure for each paid coach/trainer.  Once signed, this form is good indefinitely, and does not need to be resubmitted every year.  If your paid coach/trainer is new to the Club, email him/her the link to this page so that he/she can complete the form and mail it to our treasurer.  If your paid coach/trainer is not sure that he/she has submitted a signed form, ask him/her to email the treasurer to see if we have a form on file (you can find the Treasurer's email in the Contacts section, on the Board of Directors tab).

Valley United does not require teams to use a standard contract with their paid coach.  Each team should make an explicit agreement with their coach about rate, requirements, and pay dates, and then document that agreement as the parties desire.

PAYING THE COACH/TRAINER
Payments to the coach/trainer must come from the Club's treasurer, for tax reporting reasons.  The manager should never pay the coach/trainer from the team's account or using personal funds.  When a payment is due to a coach/trainer, the manager should send an email to the Treasurer, requesting that a check be cut and mailed.
