Pre-season paperwork for families

Before the season starts, your manager needs some paperwork from you.

(1) PHOTO
To make passes, we need a photo for each player and for each adult helper (coaches, managers).  The photo should be about the same size as the photo on your Minnesota driver's license.  You may print it on photo paper or plain paper.

(2) BIRTH CERTIFICATE COPY
The team manager will need a photocopy of your child's birth certificate, passport, or Green Card.  It does not need to be an "official copy" issued by the government, nor do you need to have the copy notarized.  A simple photocopy is sufficent.

(3) DATE CONFLICTS
Make a list of the dates when your player is not available for soccer, due to his/her own events or major family events.  This will help your manager to reschedule games and practices to nights that work best for the majority of the team.

(4) MYSA MEDICAL RELEASE
Follow the link below to get the form from the MYSA website.  This form does NOT need to be notarized UNLESS your player will be in a tournament outside of MYSA's area, which is Minnesota and western Wisconsin.

(5) VALLEY UNITED PLAYER CODE OF BEHAVIOR
Look below to find this form.  We need a freshly-signed version of this form for each season.

(6) VALLEY UNITED PARENT CODE OF CONDUCT
We need a freshly-signed version of this form each season.  If you have players on more than one team in the Club, each team manager needs to receive a copy of the Parent Code of Ethics.  If you have twins or triplets on one team, we only need one form.  

Coaches: If you are coaching on more than one team (thank you!), please prepare one code-of-conduct form for each team.

(7) BLANK CHECK
When you go to the team parent meeting, bring a blank check.  You'll need the check to pay your fair share of the team's budget.
